NCSAA Tournaments Medical Information and Waiver Forms
A completed NCSAA Tournament Medical Information and Waiver Form must be submitted for every athlete participating in any NCSAA Tournament. If the athlete is a minor (under the age of 18), the form must be completed by the athlete's parent/guardian.
No player will be permitted to participate in any NCSAA tournament if NCSAA does not have a completed, current, and valid form on file for that player. No registration fees will be refunded for any player or team that is unable to participate in part or all of any tournament due to the failure to complete and submit the Medical Information and Waiver Form. A new form must be submitted for every tournament that each player attends.
The submission of your team's Medical Information and Waiver Forms may be accomplished in one of two ways:
1) Each parent / guardian may access this page online and complete the form for his/her child. Use the links below to access the forms.
2) You may distribute this paper form to each of your athletes, and their parents/guardians may complete the form and return it to the school. Then someone from the school (coach, athletic director, etc) must enter the form online (via the online forms below), on each parent/guardian's behalf. If you submit your waiver forms in this way, you must bring the original paper form and give it to NCSAA during on-site registration (so that we will have an official copy of the signed forms).
